Participant agreement and release of liability
1. Assumption of Risk. I understand and acknowledge that participation in sports and related physical activities involves inherent risks, including but not limited to: sprains, strains, fractures, eye injuries, cardiac events, and other physical injuries. I voluntarily assume all such risks, whether known or unknown, associated with my participation.
2. Release of Liability. I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release, waive, discharge, and covenant not to sue the Ronkonkoma Sports Club, its officers, directors, employees, volunteers, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in the program or while on or about the premises.
3. Medical Responsibility. I acknowledge that the Ronkonkoma Sports Club is not responsible for any medical expenses, costs of treatment, or related charges incurred as a result of injury or illness sustained during participation. I agree that I am solely responsible for obtaining and maintaining my own health insurance and for all costs related to any medical care I may require.
4. Medical Authorization. In the event of an emergency where I am unable to make decisions for myself, I authorize the Club staff to contact emergency medical services and to provide my emergency contact with relevant information. This does not obligate the Club to bear any medical costs.
5. Conduct and Rules. I agree to comply with all rules, regulations, and instructions issued by Club coaches and staff. Failure to comply may result in removal from the program without a refund.
6. Photo & Media. Photography and video recording may take place at all Club events and activities for promotional and educational purposes. By participating, I acknowledge that the Club is not responsible for capturing or removing individuals from photos or videos. If I or my child do not wish to appear in photos or videos, it is my sole responsibility to avoid areas where recording is taking place. The Club will not be held liable for any incidental appearance in photographs or video footage taken at Club events.
7. Acknowledgment. I have read and fully understand the contents of this agreement. I am signing this agreement freely and voluntarily. I am 18 years of age or older, or if under 18, this form is being signed by my parent or legal guardian.
